Who are the Children with Special Health Care Needs (CSHCN)?
Children who have, or who are at an elevated risk of, chronic physical, developmental, behavioral or emotional conditions (biologic or acquired). These children also require health and related services (not educational or recreational) of a type and amount not usually required by children of the same age.

How can the YCCSHCN help?
Our Center works in collaboration with various Yale University School of Medicine programs and the Connecticut Departments of Public Health and of Social Services to provide resources and support services reflecting the highest standards of family-focused, community-based and culturally-sensitive care. All families of the children described above, regardless of income, are eligible to access our network of professionals and parents who can offer:

  • Clinical Expertise
  • Community Care Coordination
  • Lending Library
  • Parent to Parent Support
  • Referrals and Resources

Our goal is to help families, specialists and primary care providers throughout Connecticut ensure that children with special health care needs receive comprehensive, coordinated care.

Title V Children with Special Health Care Needs Program
Children who meet the medical definition of CSHCN are eligible to participate and benefit from Care Coordination and Family Support Services offered at the Yale Center. Our staff will work directly with you and your child's medical home. Call the Center directly to learn more on how we can assist you and your family.

Importantly for more eligilble children, the Title V CSHCN Program offers payment for certain types of services including, but not limited to:

  • Adaptive and specialty equipment
  • Hearing Aids
  • Medical and/or Surgical Supplies
  • Specialty medical, pharmacy and special nutritional formulas
  • Respite
